Thomas J. Ptacek, age 76, of Prescott, Wis., formally of Portland, Ore., passed away on Tuesday, Dec. 23, 2025 at Comforts of Home in Hudson, Wis. Tom was born on Nov. 8, 1949, in Prescott, Wis. to Donald and Charlotte (Farrell) Ptacek.
Tom was proceeded in death by his parents and his sister Mary Jo Rusch. He is survived by his brothers Michael (Jan) Ptacek and Peter (Joan) Ptacek; brother-in-law, Lee Rusch; and many nieces and nephews. A private family funeral was held on Jan. 8, 2026 at St Joseph’s Catholic Church Prescott.
A celebration of life will be held from 1-5 p.m. Monday, March 23, 2026 at The Old Ptacek’s Event Center, 1449 Orrin Rd., Prescott, WI.
